Transient functional improvements in advanced Alzheimer's disease following high-dose psilocybin: a case report

Summary

This case report describes an 80-year-old Japanese-American woman with advanced Alzheimer's disease (10-year history, 5 years of severe hypofunction and monosyllabic speech) who received a high dose of psilocybin-containing mushrooms. The treatment resulted in transient but notable multidomain functional improvements, including increased verbal fluency, improved emotional expression, enhanced mobility, and regained ability to perform daily activities. The improvements lasted approximately 3-4 weeks before gradually returning to baseline. The case suggests that psilocybin may transiently alter brain network dynamics and induce plasticity-related mechanisms even in advanced neurodegenerative disease, challenging the assumption of irreversible decline in late-stage Alzheimer's.
