Summary
Researchers developed an artificial exosome nano-decoy (AT@NV-PD1) that targets senescent-like neutrophils in sepsis. This nanotherapeutic approach simultaneously addresses both hyperinflammation and immunosuppression in sepsis by inducing timely apoptosis of dysregulated neutrophils (reducing cytokine storms) and blocking the PD-1/PD-L1 immune checkpoint (mitigating immunosuppression), thereby restoring immune homeostasis. The engineered nano-decoy uses a pH-responsive bovine serum albumin core to home to specific immune cells and deliver a dual therapeutic effect.

· 5 pulledA critical challenge in sepsis treatment lies in its complex immune microenvironment, characterized by concurrent hyperinflammation and immunosuppression.
This imbalance is jointly driven by dysregulated neutrophil programmed death and abnormal activation of the PD-1/PD-L1 immune checkpoint.
Precisely modulating neutrophil fate and blocking this immune checkpoint are highly promising therapeutic strategies.
It alleviates the cytokine storm by inducing timely apoptosis of these cells and neutralizing toxins; concurrently, it mitigates immunosuppression by blocking the PD-1/PD-L1 immune checkpoint.
Ultimately restoring the body's immune homeostasis.
