This cohort study analyzed data from 586,936 US adults (National Health Interview Survey, 1997–2018) to examine how different combinations of moderate-intensity physical activity (MPA) and vigorous-intensity physical activity (VPA) are associated with all-cause, cardiovascular, and cancer mortality. The study found that any combination of MPA and VPA providing 150–300 minutes of MVPA per week was associated with substantial mortality reductions. Notably, participants who engaged in some VPA (even as little as 25–50 minutes per week) achieved similar or greater benefits compared to those doing only MPA, suggesting that incorporating vigorous activity may allow for more efficient exercise regimens. The optimal risk reductions were observed at 150–225 minutes of MVPA per week, with VPA contributing additional benefits beyond MPA alone.
