Summary
A 79-year-old man developed delayed hemichorea-dystonia (involuntary movements) several days after a right carotid endarterectomy for severe internal carotid artery stenosis. The initial post-surgical complication was acute left-sided hemiplegia, with MRI showing a punctate ischemic lesion in the right precentral gyrus. The delayed onset of hemichorea-dystonia represents an unusual neurological presentation following cortical stroke from carotid intervention.
